Each person applying for a death certificate needs to fill out an application form.
Where to get the form?
Forms can be obtained at the main office in Kingstown or post offices and revenue offices.

Where to get the death certificate?
Death certificates can be obtained at the Civil Registry office in Kingstown
What's the procedure at the registry?
- Fill out the application form and get it certified if you had not already done so.
- Give application form to computer clerk.
- Pay $15.00 at the cash desk when the application form is returned to you.
- Collect certificate at the cash desk when your name is called.
Overseas persons
Applications are accepted in writing to the Registrar with the following information:

Further, a bank draft to cover the cost of the certificate (E.C.$15.00), bank charges and postage, should be included. The money order should be payable to "The Accountant General". Ensure that a return address is submitted.
