Local Persons

Each person applying for a death certificate needs to fill out an application form.

Where to get the form?

Forms can be obtained at the main office in Kingstown or post offices and revenue offices.

Information required to obtain a death certificate( as indicated on application form)

  • Christian names of deceased
  • Surname of deceased
  • Date of death
  • Place of death
  • Name of Applicant
  • Identification number of applicant
  • Address of applicant
  • Contact Number of applicant
  • Relationship of applicant to the person whose death certificate is required

Cost for death certificate

Each death certificate cost EC $15.00

Where to get the death certificate?

Death certificates can be obtained at the Civil Registry office in Kingstown

What's the procedure at the registry?

  • Fill out the application form and get it certified if you had not already done so.
  • Give application form to computer clerk.
  • Pay $15.00 at the cash desk when the application form is returned to you.
  • Collect certificate at the cash desk when your name is called.

Overseas persons

Applications are accepted in writing to the Registrar with the following information:

  • Name of deceased
  • Date of death
  • Place of death
Further, a money order to cover the cost of the certificate (EC $15.00), and postage, should be included. The money order should be payable to "The Accountant General".  Ensure that a return address is submitted.
