Local Persons

Each person applying for a birth certificate needs to fill out an application form.

Where to get the application form?

Forms can be obtained at the main office in Kingstown or district post offices and revenue offices throughout the country.

Information required to obtain birth certificates (as indicated on form)

  • Christian names

  • Surname

  • Date of Birth

  • Place of birth

  • Mother's name

  • Father's Name (where applicable)

  • Name of Applicant

  • Identification number of applicant

  • Address of applicant

  • Contact Number of applicant

  • Relationship of applicant to the person whose birth certificate is required

 ALL APPLICATION FORMS MUST BE CERTIFIED.  PLEASE NOTE THE LIST OF PERSONS WHO CAN DO SO BELOW:

1. Justice of the Peace
2. Notary Public
3. Magistrate
4. Marriage Officer
5. Lawyer/Solicitor
6. Member of Parliament
7. Police Officer of Gazetted Rank

Who can apply for a Birth Certificate?

  1. The owner of the birth certificate. He/she must present their Identification Card.
  2. The parents whose names are shown on the birth certifcate. He/she must present their Identification Card.
  3. Children of the owner of the Birth Certificate. He/she must present their Identification Card.
  4. Legal Guardian. A court Order must be shown.
  5. An attorney acting on behalf of his client whom she/he knows for a minimum period of five (5) years. The attorney must provide: 
    • A letter of authorisation signed by the owner of the birth certificate,
    • A letter stating that he/she is acting for the owner of the Birth Certificate and authorizing their Clerk to apply for and collect the Birth Certificate
  6. Social Welfare Officers, Police Officers etc. A letter must be sent by these departments.
  7. A duly authorized person by the applicant. The applicant must submit:
    • A letter of authorisation signed by the owner of the birth certificate,
    • A copy of the owner of the Birth Certificate Identification Card
    • If the owner of the Birth Certificate is living abroad a letter of authorisation is required which must be notarized

Where to obtain certificates?

Certificates can be obtained at the Civil Registry Department in Kingstown.

Cost for birth certificate

Each birth certificate cost EC $15.00 and are issued immediately.

What's the procedure at the registry?

  • Fill out the application form and get it certified if you had not already done so.
  • Give application form to computer clerk.
  • Pay EC $15.00 at the cash desk when the application form is returned to you.
  • Collect certificate at the cash desk when your name is called.

Applications are accepted in writing to the Registrar with the following information:

  • Mother's name

  • Date of birth

  • Place of birth

  • Person's name

The letter of application must be signed by the applicant, and notarized by a Notary Public. In addition, a copy of the owner of the Birth Certificate Identification card.

Further, a money order to cover the cost of the certificate (E.C.$15.00), and postage, should be included. The money order should be payable to "The Accountant General". Ensure that a return address is submitted.
